Dr. Ralph does not contract with any insurance plan. In other words, she does not accept or bill insurance for payment of services. You will be responsible for paying the full fee for services.

You should contact your insurance plan to find out:

  1.  if you have out-of-network benefits

  2.  how much you may be reimbursed after you meet the out-of-network deductible.

You will need to ask your insurance company about coverage for psychological testing, in particular. The relevant billing codes, or CPT codes, for the testing process are:

  • 90791

  • 96130

  • 96131

  • 96136

  • 96137

In addition, we recommend checking whether your health plan will cover testing for your specific areas of concern (e.g., testing for dyslexia or ADHD).  We provide itemized receipts (also called a superbill) to make the process of seeking reimbursement from your insurance company easier for you. If you wish to have your insurance company reimburse you, you will need to complete your health plan's claim form and send a copy of the receipt.

You can also consider using the funds from your health savings account (HSA) / flexible spending account (FSA) or ask a tax professional if you can deduct a portion of the fees from your income taxes.

TRICARE: Dr. Ralph is an authorized, non-network, non-participating provider. You may consider reading over TRICARE’s explanation here.

Medicaid or Medicare:  Dr. Ralph does NOT accept Medicaid or Medicare and does not submit any bill (medical claims) to Medicare/Medicaid. As such, services through Lifespan are not subject to the Medicare law that limits the amount doctors may charge patients.
